 How to Apply

HOW TO APPLY

To apply:

                                 Phone Meals on Wheels office: (716) 693 – 1663

                                 9:30 AM – 1:30 PM, Monday – Friday (except holidays)

You will be interviewed by Joy Welch, NTMOW Coordinator / Director and asked to answer a few questions to start meal deliveries. She also forward your information to the NTMOW Case Manager, a Registered Nurse who also has additional counseling training.

The NT Meals on Wheels Case Manager will:

1)     Make an appointment to visit each new client in their home

·        family members are encouraged to be present

·        review allergies, health and medical restrictions

2)     Assess the needs of the client’s meal program

·        explore nutrition and dietary issues

·        check that the client has adequate health / medical insurance

·        provide information for food stamps to pay for the program, Medicaid and other agencies where necessary             

3)     Discuss who helps them around the house, getting to and from appointments, cutting their lawn, and shoveling their sidewalks      

All information is confidential, submitted as a written report and reviewed with the Meals on Wheels Director. It’s kept in a file for future reference, if needed.
